OneForth LLC ("OneForth," "we," "our," or "us") is a wound care marketing services company that helps wound care providers acquire qualified patients. This Privacy Policy describes how we collect, use, disclose, and protect personal information that comes through our website, our discovery and onboarding processes, and our care coordination operations. By engaging with us, you agree to the practices described here.
2. Information We Collect
We collect information from two distinct sources, and we treat each accordingly.
From practice principals and prospective clients via our website forms, email, and discovery calls — typically name, practice name, role, business email, business phone, service area, and information you choose to share about your practice and goals.
From patients via our strategic media partners, only with the patient's affirmative consent. This information is governed by HIPAA and our internal HIPAA compliance practices, and is not used for marketing.
3. How We Use Your Information
We use the information we collect to respond to your inquiries, schedule discovery calls, deliver our patient acquisition and care coordination services, communicate with you about your engagement, improve our service offerings, and meet our legal and contractual obligations. We do not use your information to train third-party advertising models.
4. HIPAA & Protected Health Information
Patient information collected through our care coordination process is treated as Protected Health Information (PHI) under HIPAA, regardless of whether each specific data point would technically qualify. We maintain Business Associate Agreements (BAAs) with our strategic media partners and with every wound care provider before any PHI is exchanged. For full details, see our HIPAA Compliance page.
5. Information Sharing
We do not sell your information. We share patient information only with the wound care provider the patient has consented to be introduced to, and with our care coordination partners and subcontractors operating under a BAA. We may disclose information when required by law, subpoena, or to protect the rights and safety of OneForth, our clients, or the public.
6. Your Rights
You have the right to access the information we hold about you, request corrections, request deletion (subject to legal and operational retention requirements), and opt out of non-essential communications. Patients additionally have HIPAA-specific rights described on our HIPAA Compliance page.
7. Cookies & Tracking
Our website uses standard analytics cookies to understand aggregate traffic patterns and improve performance. We do not run targeted advertising or behavioral re-marketing on this site. You can disable cookies in your browser settings without losing core functionality.
